01Photon absorptionv
Red and near-infrared photons penetrate skin and are absorbed by chromophores — light-sensitive molecules inside cells. The primary chromophore is cytochrome c oxidase (CCO), located in the inner mitochondrial membrane. CCO absorbs specific wavelengths — particularly 630nm, 660nm, 810nm, 830nm, and 850nm.
02Electron transport chain activation^
When CCO absorbs photons, it accelerates the electron transport chain (ETC) — the series of protein complexes in the mitochondrial membrane that shuttle electrons and pump protons across the membrane. This creates a stronger electrochemical gradient — the proton motive force that drives ATP production.
03ATP synthesis increasesv
The enzyme ATP synthase uses the proton gradient to produce ATP — the universal energy currency of the cell. More photon absorption means a stronger proton gradient and more ATP produced. Studies have shown 30-40% increases in cellular ATP output following photobiomodulation sessions.
04Downstream cellular effectsv
More ATP enables cells to do more work faster: fibroblasts produce more collagen, immune cells regulate inflammation more efficiently, muscle cells clear metabolic waste and synthesise new protein. This is why RLT has effects across such a wide range of systems — the mechanism is upstream of almost everything.

Cytochrome c oxidase (CCO) is Complex IV of the mitochondrial electron transport chain. It is the primary photoacceptor in photobiomodulation — the molecule that actually absorbs the red and NIR photons and converts that energy into accelerated electron flow.

Under normal conditions, CCO can become partially inhibited by nitric oxide (NO) — particularly in cells under oxidative stress or low energy demand. Red and NIR light photodissociates this NO inhibition, freeing CCO to operate at full capacity.

The result: the electron transport chain runs faster, the proton gradient builds higher, and ATP synthase produces more ATP per unit time. This is not a placebo mechanism — it is measurable, reproducible, and well-characterised in peer-reviewed biochemistry literature.

The analogy
"Think of nitric oxide as a finger on the off switch of your cellular power plant. Red light removes that finger — not by adding energy from outside, but by releasing the brake that was already there. Your mitochondria were always capable of producing more ATP. The light just lets them do it."
